NOTICE TO KRISTA PLATSKO This notice is given by virtue of an Order for Substitutional Service pronounced on October 7, 2015 by Master J.T. Prowse. A statement of claim has been issued against you as of July 21, 2015, in the Judicial District of Calgary, as Action Number 1501-08324. The Statement of Claim alleges that Ngoc Tom Dinh, Cecile Linh Hai Ngo, and Phyc Dinh were passengers in a 2001 Hyundai motor vehicle bearing Alberta license plate BDB 4026, which was invoved in a motor vehicle accident on August 8, 2013 on 14th Street SW, at or near the intersection of 8th Avenue SW, Calgary, Alberta. This resulted in personal injuries to the occupants of the vehicle you struck when your vehicle struck the plaintiff's vehicle.
